Antibiotics for prevention of periprosthetic joint infection following dentistry: time to focus on data.

چکیده
The population of individuals with artificial joints is steadily increasing, because more devices are implanted, and the life span of people with implants is longer. During a 10-year period, the total number of knee replacements increased from 270,000 to 550,000 per year, and the total number of total hip replacements increased from 120,000 to 200,000 per year in the United States [1]. Prosthetic joint– associated infection (PJI) is a devastating complication that occurs in 0.3%–1% of those patients who undergo total hip arthroplasty and in 1%–2% of patients after total knee arthroplasty [2]. Of these episodes, 35%–40% occur by the hematogenous route [3]. Most of these episodes are sequelae of Staphylococcus aureus sepsis, skin infection, or urosepsis [4–6]. It is conceivable that a small portion of these PJI episodes are caused by transitional bacteremia during dental work. However, clinical experience does not favour this hypothesis, and to date, only few indirect data were available for discussion.

EDITORIAL Table 1 summarizes recommendations based on the presented papers. To prevent infection, it is critical to identify high-risk patients and control those risks prior to surgery. Action must be taken at every step of the surgical process, from preoperative to postoperative protocols, including intraoperative measures. متن کاملPrevention of Periprosthetic Joint Infection
Prosthetic joint infection (PJI) is a calamitous complication with high morbidity and substantial cost. The reported incidence is low but it is probably underestimated due to the difficulty in diagnosis. PJI has challenged the orthopaedic community for several years and despite all the advances in this field, it is still a real concern with immense impact on patients, and the healthcare system....
متن کاملIncreased Risk of Heterotopic Ossification Following Revision Hip Arthroplasty for Periprosthetic Joint Infection
Purpose: To investigate whether surgery for PJI of the hip, the number of procedures and their duration contribute to risk of HO formation. Methods: 56 patients with hip PJI undergoing one-stage (10) or two-stage (46) exchange arthroplasty were matched to 112 patients undergoing revision arthroplasty for aseptic failure based on age, gender, body mass index (BMI), surgical approach (all direct...
متن کاملThe Zeitgeist of Challenging the Evidence. A Perspective on the International Consensus Meeting on Periprosthetic Joint Infection
Background: The economic burden of the treatment of periprosthetic joint infection (PJI) is high and the treatment of PJI has a high degree of international controversy. Several papers have declared the International Consensus Meeting on Periprosthetic Joint Infection (ICMPJI) to be the "flawless pledge of international academics" to overcome the challenges of musculoskeletal infections. The p...
